Why this role is important?

The Nurse Practitioner Lead is a licensed Nurse Practitioner in Canada who provides virtual health care, education, and support to patients in accordance with applicable provincial licensing requirements and regulations governing cross-jurisdictional care. 

In this role, you will combine direct patient care with clinical and team leadership. As a key point of contact for patients, you will provide convenient, timely, and patient-centred virtual care ranging from the assessment and treatment of minor acute conditions to health coaching and ongoing support for a variety of health needs. 

You will assess and triage patient concerns, develop appropriate clinical recommendations and care plans, prescribe and evaluate treatments within your scope of practice, and support patients in achieving improved health outcomes. Care may be delivered through multiple virtual channels, including secure messaging, phone, and videoconference. When appropriate, you will also refer patients to other health care providers, facilities, and community-based resources. 

As the Nurse Practitioner Lead, you will provide day-to-day leadership and clinical support to a team of Nurse Practitioners. You will serve as a clinical subject matter expert, support quality and program improvements, contribute to policy development, and help build the knowledge and capabilities of the NP team. 

Working collaboratively with Registered Nurses, Registered Dietitians, Pharmacists, and other health care and operations professionals, you will help deliver a seamless and meaningful patient experience while using your clinical judgment and critical thinking skills to develop, implement, and evaluate comprehensive care plans. 

What You Will Do 

  • Provide high-quality, evidence-based virtual care to patients across a broad range of health conditions and clinical needs. 

  • Assess and triage patient concerns, establish clinical diagnoses, develop treatment plans, prescribe within scope of practice, and evaluate patient outcomes. 

  • Educate, counsel, coach, and advocate for patients while demonstrating compassion, empathy, and a commitment to promoting healthy lifestyles. 

  • Apply nursing processes, critical thinking, clinical judgment, problem-solving, and evidence-based decision-making to patient care. 

  • Manage and provide day-to-day leadership to a team of Nurse Practitioners. 

  • Support onboarding, knowledge development, and ongoing clinical education for new and existing Nurse Practitioners. 

  • Provide guidance and support for day-to-day clinical questions and escalations. 

  • Act as a front-line clinical subject matter expert for quality improvement initiatives, program enhancements, and new clinical services. 

  • Contribute to the review, development, implementation, and communication of clinical policies, procedures, and best practices. 

  • Provide clinical coverage and support for NP accountabilities during planned absences and as operational needs arise. 

  • Participate in the ongoing development and improvement of the virtual care program, including collaboration with patients, health care providers, and internal stakeholders. 

  • Champion the virtual care program and identify opportunities to enhance clinical quality, patient experience, and operational effectiveness. 

  • Refer patients to appropriate health care providers, facilities, specialists, and community-based resources when required. 

  • Ensure care is delivered in accordance with applicable professional standards, organizational policies, privacy requirements, and provincial and federal legislation. 

  • Maintain direct patient care accountabilities through scheduled appointments and protected time dedicated to leadership, clinical support, quality improvement, and program responsibilities. 

  • Additional duties or tasks may be assigned as operational needs evolve. 

What You Will Need 

  • Current, unrestricted Nurse Practitioner registration and authorization to practise in the province of employment. 

  • Willingness and ability to obtain licensure in additional Canadian provinces as required to support program services. 

  • A minimum of two years of direct clinical experience as a Nurse Practitioner is preferred. 

  • Demonstrated leadership, mentorship, or people-management experience is preferred. 

  • Strong clinical expertise and experience assessing and managing a broad range of health conditions. 

  • Strong ability to assess, diagnose and develop and patient management. 

  • Strong knowledge of pharmacology, pharmacokinetics, and pharmacodynamics, with the ability to apply this knowledge to prescribing and evaluating treatment response. 

  • Excellent assessment, communication, critical thinking, clinical judgment, and decision-making skills. 

  • Ability to provide organized, efficient, and patient-centred care in a virtual care environment. 

  • Demonstrated understanding of organizational policies, professional standards, legal considerations, privacy requirements, and professional ethics. 

  • Strong interpersonal skills and the ability to collaborate effectively as part of an interdisciplinary health care team. 

  • Ability to remain objective, adaptable, and effective in a fast-paced and evolving environment. 

  • Strong commitment to quality, patient-centred service delivery. 

  • Proficiency with Microsoft and web-based applications; experience with Salesforce or similar platforms is an asset. 

  • Excellent verbal and written communication skills in English; bilingual French/English capability may be required for certain roles or jurisdictions. 

  • Availability to work a flexible schedule that may include days, occasional evenings and weekends, as well as potential on-call shifts.
